Changes to SACES selection
This form is not intended to replace the general SACES selection process, which currently collects instructors' opt-in / opt-in selections on the Instructor Course Description Form accompanying student evaluations. Rather, it is intended to be used by faculty who wish to update or correct selections indicated in past terms. Please review the Office of Assessment's policy regarding edits to previous terms' SACES selections.
If you wish to change the SACES status of multiple courses, please submit separate requests via this form. After you click submit at the end of the survey, you will have the option of returning to this screen to submit an additional request.
All information requested on this form is required. If you have difficulty finding any of the required information, please consult your department's or program's DUS assistant.
